A Guide to SEAS “G” Forms
If you want [Column A], then complete the forms in [Column B].
 
MS/ME: Submit a plan of study outlining 30 credit hours of course work and research required to graduate. For ME programs indicate which assessment forms are required.
(may be re-submitted if changed)
Submit each assessment forms as it is completed.
Ph.D.: Submit a plan of study outlining 72 credit hours of course work and research required to graduate.
(2 pages/may be re-submitted if changed)
Ph.D.: Appoint your advisory committee.
(may be re-submitted if changed)
MS/ME and Ph.D.: Identify a faculty member as your academic advisor.
MS and Ph.D.: Request any graduate examination (MS final, Ph.D. Qualifying, Dissertation Proposal or Final).
Ph.D.: Submit a report of your qualifying exam. Also submit the program-specific G107 assessment form.
and G107xx (for each program)
Ph.D.: Submit documentation of dissertation proposal, to be completed at the “Proposal Examination.”
MS: Submit a report of Final Examination, to be completed at exam.
Ph.D.: Submit a report of Dissertation Final Examination, to be completed at exam.
MS/ME: Record transferred credits/courses—will appear on U.Va. transcript (limit 6 in MS/12 in ME).
MS/ME and Ph.D.: Apply for graduate degree (submit by 10/1, 2/1 or 6/1) for graduation at semester’s end.
MS: View thesis preparation Checklist.
MS/ME: Request to continue from Master’s program to Ph.D.
